Yours is mounted correctly, mine was one notch too far back. I had to move it fwd. Have you checked your bcg?

Some variants of the Swan Sleeve were used on the Mk12, which is kind of why I wanted one. I paid $300 for my upper with the sleeve and a Colt bcg, I believe the rest of the parts are Bushy. I think the main reason for the sleeve was to use it as a 1/2" riser but that is just pure speculation...
